Volunteers of America of Minnesota geriatric care managers offer high caliber, intensive care management and consultation services for older adults and their families. We provide objective multi-dimensional assessments that clarify an elder’s needs and reduce conflict. We have been helpful to families, attorneys, trust departments and guardians/conservators in providing short-term consultation services and ongoing monitoring and service coordination.
Our care managers have over 50 years of combined experience working with cognitively impaired clients and their families and specialize in working with families and individuals who have been impacted by dementia. We work to balance respect for self-determination with concerns for safety and always honor an older person’s rights, values and lifestyle.
As part of a diverse organization providing services to seniors, we are uniquely able to utilize our colleagues’ expertise in areas such as nursing and mental health to assist in developing appropriate care plans.
Volunteers of America of Minnesota’s nonprofit status offers our care managers the ability to focus specifically on the participants’ needs rather than on revenue generated. Our goal is to ensure our clients’ safety, health and overall well-being without fostering unnecessary dependence on the geriatric care manager’s services.
A comprehensive written report is available at an additional charge. This report may provide valuable information for the older adult’s physician, out-of-town family members who may not currently understand their loved one’s needs, and/or prospective care providers. Many people have found the written reports are helpful in resolving family conflicts about the older adult’s needs, or in contested court cases where an objective assessment and recommendations are needed to help a judge make a legal determination.
Short-Term Consultation: Volunteers of America of Minnesota Care Management and Consultation's geriatric care managers can meet with family members to review an older adult’s situation and provide consultation regarding services, housing and care options.
Ongoing Monitoring and Service Coordination: Volunteers of America of Minnesota Care Management and Consultation geriatric care managers ensure that the older adult’s needs are met on an ongoing basis. As care needs change, the care manager will reassess and make recommendations about how to adjust the care plan for the older adult to make certain that their needs are being met.
Dementia and End of Life Care: Volunteers of America of Minnesota Care Management and Consultation's geriatric care managers have extensive experience working with older adults who have cognitive impairment and who may lack insight into their own needs. Our care managers can serve as the firm but respectful voice that works with an older adult who is resistant to changes necessary to ensure his/her health and safety. Additionally, our care managers also have expertise in hospice and palliative care and can support and inform families as they sort through the options available for end-of-life care.
